To complete your provider verification, please keep the following documents ready:

• Aadhaar Card (front & back)
• PAN Card
• Recent passport-size photograph
• Relevant certificate — nursing council registration, GNM/ANM/BSc Nursing diploma, physiotherapy license, or caretaker training certificate (where applicable)
• Police verification certificate, or consent to undergo one via our verification partner
• Proof of current address (utility bill or rental agreement)
